A real exorcism usually occurs in a church and is done by a trained Catholic priest and his team, often including a strongman, with the permission of the bishop and only after a thorough medical exam and psychological/psychiatric exam rule out such causes.
An exorcism is a sacramental, not a sacrament, for the care of souls, even souls not of the flock. The following document from the US Conference of Catholic Bishops outlines what it is and how it is to be executed.
